Please complete this form if your child is a member of Aylsham High School's Friendly Faces team. The Friendly Faces team support at various school discos across the year and we would like your consent for your child to assist at these events.

I understand that my child will need to be at school by 6.30pm on the evenings of these discos.

I understand that my child will be ready to be collected from school at 8.30pm following the discos.

I will ensure that I have made arrangements for my child to get home safely following the discos.
